Zueike Medical & Liability Agreement

I acknowledge that I fully understand the purpose of this program, the type of activities that me and/or my child will be participating in, and that I have received and reviewed the basic program schedule and layout. I understand that there are inherent risks with organized games and activities and I accept these risks. I agree to release, discharge and hold harmless Zueike, its staff and agents and the program named above from and against all claims and proceedings with respect to any damage or injury to myself, and/or my child, and/or my property, arising from the provision of these services and activities. If I cannot be reached, I authorize Zueike staff to consent to emergency medical treatment as needed, with any related expenses being my responsibility.

I have read, understood and agree to the terms and conditions of the above Zueike Medical & Liability Agreement.

Zueike Social Media & Marketing Release Agreement:

I hereby grant permission for photographs, video recordings, and/or digital images of me and/or my child to be taken during events, activities, or programs organized by Zueike. I authorize these images to be used for promotional materials, including but not limited to websites, social media, printed publications, and press releases. I understand that names will not be used without additional written consent. I agree that all such photographs and recordings shall remain the property of Zueike, and I waive any right to inspect or approve the finished product. I release Zueike and its representatives from any liability connected with the use of these images.
